Customer Loyalty Rebate Form I agree to buy Thermocraft Products exclusively and in so doing am entitled to a 5% "End of Year" Rebate based on my yearly online minimum purchases of $3,000.00 from January through December. The monthly specials and closeouts purchases cannot exceed 20% of total to qualify. Freight, returns and local taxes where applicable will be deducted prior to applying rebate.
